Medical Records
Pana Community Hospital’s Medical Records Department is committed to fulfilling all requests for medical information. To obtain a copy of your medical records at Pana Community Hospital, you must submit a written, signed and dated authorization form. Upon receipt of the completed authorization form, our goal is to process all patient requests within 1 business day. Patients who pick up their records in person will need to bring photo identification. Nominal copying fee may apply.
The Medical Records Department is open from 7:30am to 4:00pm Monday through Thursday, and from 7:30am to 3:30pm on Friday.
