Donor Walls Policy

This policy applies to the Pana Community Hospital Foundation's Circle of Roses and Circle of Memories donor walls.

  1. The donor walls' giving level shall start at $100.

  2. The giving levels for the donor walls are

$100 - 249 Cameo Rose
250 - 499 Gala Rose
500 - 999 Minuet Rose
1,000 - 2,499 Masterpiece Rose
2,500 - 4,999 Emblem Rose
5,000 - 9,999 Royalty Rose
10,000 + Champagne Rose
  1. Once names are put on a donor wall, they stay on the wall indefinitely.

  2. Donor names will be arranged alphabetically on the donor wall.

  3. Nameplates will be prepared for the donor personally or in memory of a loved one if requested.  Only one nameplate will be prepared per gift.  It will not be possible to have a personal nameplate prepared for the donor and also a memorial nameplate.

  4. Donor gifts will accumulate annually to enable donors to move up to the appropriate giving level.  If a substantial gift is made, the Foundation will look at this gift on an individual basis and determine if that gift should be moved into the appropriate giving level sooner than on an annual basis.

  5. The donor walls shall be updated on a quarterly basis to add new donors.  This update shall be on a calendar basis with updates being completed as soon as possible after the end of March, June, September, and December of each year.

  6. New club donors will be notified of the date and time of the quarterly updates, so they may be present if they wish to see their names being placed on the donor wall.

  7. Payroll pledges and electronic fund bank transfer pledges will be placed on the donor wall at the pledge amount.  All other ledges will be placed on the donor wall at the amount of the first pledge payment.  As the pledge payments are made, the donor will move up on the wall to the appropriate giving level.

  8. Gift-in-Kind donations must be tangible items or professional services to be recognized on the donor wall.  All Gift-In-Kind donations require a receipt from the donor in order for the donor's name to be placed on the wall at the appropriate giving level.

  9. For matching gifts in which an individual makes a donation and his or her employer makes a matching contribution, the individual shall be given credit for the total amount of both gifts and the employer will be given credit for the mount of its gift.

  10. Any family that names PCH as a memorial for a loved one, and the amount given is equal to or exceeds $500, will be invited to come into the Foundation office to have their picture taken to be placed in the area newspaper or for themselves personally if they do not wish to be recognized publicly.

  11. In order for a person to be recognized in the Huber Society, all life insurance policies must name Pana Community Hospital Foundation as the owner of the policy.  In order for a person to have their name placed on the Huber Society plaque, they will be asked to sign an affidavit stating that they have left a bequest to Pana Community Hospital in their will.
